Notice is hereby given that TIFFANY DENISE TOLBERT, the undersigned, filed this petition in the Superior Court of Crisp County, Georgia on the 26th day of January, 2026, praying for a change in the name of minor child from CHANCE DEMETRIC PATRICK to CHANCE DEMETRIC TOLBERT.
